As the deadline approaches, ECFMG will cease accepting applications for the 2026 pathways on April 20, 2026. These pathways, designed specifically for International Medical Graduates (IMGs), provide a critical opportunity for aspiring physicians to achieve ECFMG certification and realize their dreams of practicing medicine in the USA. DHS Diverse Health Solutions LLC encourages IMGs to take proactive steps by creating an account at MyIntealth.com for updates on the new 2027 pathway. It’s essential to verify that your medical school is listed in the World Directory of Schools of Medicine and to gather all necessary documentation for submission. Additionally, prepare for the Occupational English Test (OET), ensuring a score of 350 in listening, reading, and speaking, and 300 for writing. Stay informed and ready as the end of the 2026 pathways approaches.

Patients who participate in clinical trails have a possibility to obtain a health improvement individually, their participation will benefit health status in future patients. Some of the newest, most innovative cancer treatments are available only through clinical trials. Those require approval by an ethics committee, requires oversight with regard to compliance with the protocol, and involves the support of extra research staff in the monitoring of care, although a monetary compensation for the time, and a better understating of their disease and diagnosis.

Heparin (prophylaxis): 5000 Unit (fixed) SC every 8 or 12 hours is an effective and safe form of prophylaxis in medical and surgical patients at risk of venous thromboembolism. Low-dose heparin reduces the risk of venous thrombosis and fatal PE by 60% to 70%(Anticoagulation guideline)

My name is Joel Collazo. I am a medical doctor from Havana, Cuba. I left my natal country about 4 years after I graduated and when I was about to finish my specialization in Histology. I lived in Peru, where I practiced medicine and did ultrasound. In 2007, I moved to the United States with the idea of either obtaining the license to practice medicine or studying nursing. However, I always loved teaching and I did have the opportunity to begin teaching in 2009 at Miami Dade College (MDC) as an adjunct faculty for anatomy & physiology, and biology. At some point I started thinking about obtaining an “American diploma” that qualified me to get a job in an “easier” way and it was while I was working at a private college while still at MDC that I was “required” to have a master’s degree, which I completed at Barry University. I did it and right after finishing, a friend and colleague told me and some other friends/colleagues about a Ph.D. program in Public Health with specialization in epidemiology, fully online, and without too many pre-entry requirements like exams, etc.

I decided that that could be an excellent opportunity for me to earn a terminal degree that would help me further my job opportunities and at the same time, give back to the country that I was so grateful for. The master took me less than two years, but the Ph.D. took me 8 ½ years. Here comes my advice, never give up although the horizon looks so far that is invisible. My doctoral dissertation was my first research ever. It took me about 3 years to fine tune my research topic because I did not know how to organize, prepare, and conduct a secondary data analysis. This frustrated me because I had an approved prospectus on Epigenetics and Epidemiology of Breast Cancer however, I did not have data. I thought about giving up sometimes but my desire to demonstrate to myself that it is never late in life was bigger than anything else. Also, I had similar feelings regarding my family, especially my daughter, and then my students. I always tell everyone that it does not matter what or how you will be successful, if you do the right thing. Bottom line, you are in a new country, and you want to succeed. That is why you came here, and as a professional in your country, it may happen that you need to adjust, or even change that profession. Maybe you were not a professional but now you have decided what you want to be. Whatever your situation is, do not give up. It is not easy to be reborn, but when you are, the opportunities can be endless.

Being a registered nurse (RN) in the USA is one of the most interesting, fascinating jobs, and it's a health career with wide of possibilities. According to the U.S Bureau of Labor Statistics (2024), registered nurses are one of the most rapid evolving professions (6 percent from 2022 to 2032), quicker than average over all in relation to its dynamic, annually new opened positions and broaden horizons in its proper development. Being a registered nurse (bachelor's degree in nursing, an associate degree in nursing, from a recognized approved nursing program) means that this professional can work in diverse medical and nursing settings. Some of these settings include, but are not limited to, private companies, public health departments, school nursing program, hospitals, occupational, research, education, legal offices, health insurances, and your own business. During 2022, almost 60% of the RN jobs placement were hospitals, 18 percent worked in ambulatory healthcare facilities (traveling nurses, physician's offices, outpatient care centers, and home health cares), 5 percent worked in the public or government sites, and approximately 3 percent in educational settings.

Let us share our experience: When we arrived to the USA, we left behind but inside of us our clinical, and professional experience as a physician. Here, in the USA there are diverse mechanisms to start and pursue a similar passion.

We have fallen in love with the research jobs and the registered nurse wide scope of practice. This profession demands high knowledge and passion. Here, in the USA, the RN works in the first line contacting to the patient, and its family. The RN is a path to continue the health education. When we talk with our nursing students, our hearts, souls and eyes denounce how we love this profession in this country. It is noteworthy to mention that we were physicians in our home country for roughly 11 years. We also worked in medical mission with passion, but some economical, social and political reasons in our former country, Cuba, interfered with this. In the USA, we have worked in different settings such as hospitals, research center and public health centers, private companies, and educational places. We have enjoyed all of these amazing experiences and hope to share our knowledge to other health care professionals, students and others. One of the most enriched opportunities has been working in the research and educational area. Both places have pushed one to continue the education, to not stop creating and dreaming.

We have learned that there different ways to impact the community and to serve others with professionalism, while one is working at the same time.

Remember: therapeutic communication/holistic approach (mental health)

Let’s talk about your feelings(Peplau, Leininger and Purnell theories)

Therapeutic communication is the cornerstone of the health professional -patient’s relationship: This requires empathy, therapeutic communication’s skills, a holistic viewpoint, evidence based practice, a cultural beliefs and values’ self awareness (“ putting in hold our beliefs and values”) and a real passion for the patient centered care.

Examples of questions on therapeutic communication for the NCLEX:

A patient or client is receiving chemotherapy and tells to the nurse" I think that may family would stop praying for me, because I am dying, and I am feeling tired about my family hope, the long therapies, etc

           a) Have you discuss this thought and feeling with your family?

            b)

You are feeling tired that your family is praying for your cure.

           c) You seems depressed that is frequent in patients with similar diagnosis.

            d) You should explain your feeling to your family.

analysis: when the nurse repeats the patient or client's statement is showing that she/he is listening and understanding.

           Option b

is more therapeutic: You are feeling tired that your family is praying for your cure (La option b es mas terapéutica y si se tiene dudas,se puede analizar cada opción como escogiendo la más terapéutica)

           Reflecting

            (refleccionando

, making observation or verbalizing the nurse's perception for example saying" you seems that your are tired, or sad...Are you uncomfortable with? ...(
